A copy of the contingency plan and all revisions to <br /> the plan shall be maintain d at the facility at all times. Copies of the contingency plan shall also be submitted to all <br /> local police departments, ire departments, hospitals and State and local emergency response teams that may be <br /> called upon to provide em rgency services. Immediately locate a copy of the contingency plan for the facility and <br /> submit a copy of the plan o the EHD and to departments that may be called upon to provide emergency services at <br /> the facility. <br /> This is a Class II violation. <br /> 113 CCR 66262.23(x)(4) Failed to send generator manifest copies to DTSC within 30 days. <br /> Generator copies of manif ast number were not sent to DTSC within 30 days of shipment of hazardous waste, per Mr. <br /> Kandathil. One generator yopy available for review, manifest 016140140JJK. The generator manifest copy shall be <br /> submitted to DTSC for every shipment on a manifest when California is either the generator state or the destination or <br /> consignment state. The g nerator manifest copy shall be mailed to: <br /> DTSC Generator Mani sts <br /> P.O. Box 400 <br /> Sacramento, CA 95812-0400 <br /> Immediately mail a copies of all hazardous waste manifests for this location and ensure that copies of all generator <br /> manifests are sent to DTS within 30 days of shipping hazardous waste. <br /> This is a Class II violation. <br /> 114 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> Per Mr. Kandathil the business has been operating at this location for"a couple, 3-4 months". At time of inspection <br /> only one hazardous waste disposal manifests was available for review. Mr. Kandathil stated others were at his office <br /> in Southern California. <br /> A generator shall keep a c py of each signed manifest for at least three years from the date the waste was accepted <br /> by the initial transporter. <br /> Within 30 days, provide the EHD with copies of all hazardous waste manifests from this location. Begin the practice <br /> of maintaining hazardous waste manifests for a minimum of three years. <br /> This is a Class II violation. <br /> Page 5 of 15 <br />
