$ The type and location of the emergency <br />incident; and <br />$ Evacuation of the site and work area <br />All facility personnel shall stay clear of the incident <br />area and shall immediately evacuate the site and <br />work area in an orderly manner when directed to do <br />so by the EC. <br />$ Immediately notify external emergency <br />response agencies of the Level 2 or Level 3 <br />emergency incident by dialing "911 " and <br />report the conditions of the Level 2 or Level <br />3 emergency incident. <br />$ The EC shall not delay in the evacuation of <br />facility personnel in an effort to make all <br />notifications, unless the circumstances <br />permit. <br />$ At no time shall any facility personnel place <br />him/herself or others at risk in an effort to <br />mitigate and control an emergency incident. <br />b. Maintain a written record of any incident reported to the EC, <br />including, at the minimum, the following: <br />I. Time, date and location of the Incident <br />ii. Cause of Incident <br />iii. Incident description and classification <br />iv. Notification information (i.e, who was notified, by whom and <br />at what time) <br />V. Response actions <br />8.4.3 Emergency Response Equipment <br />1.
