Oral notifications shall be made immediately to the following external agencies: <br />a. Office of Emergency Services (OES) in the event the incident requires evacuation <br />of the facility and/or surrounding area; <br />b. National Response Center (NRC) in the event the incident results in <br />the release of a CERCLA listed material which equals or exceeds <br />federally mandated reportable quantities; <br />C. California Occupational Safety and Health Administration (CAL/OSHA) <br />in the event the incident results in injury to site personnel; <br />d. The Regional Water Quality Control Board (RWQCB) must be notified <br />whenever there has been a release of a hazardous material or waste <br />into a waterway (i.e, pond, lake, river, canal, ditch, storm drain, creek <br />or ocean); and <br />e. External local emergency response agencies and services (private) <br />in the event of a Level 2 or Level 3 incident. <br />4. QBR's Plant Manager, or the designated representative, shall be responsible <br />for all incident reporting and notifications. Agency notifications shall be made <br />as soon as: <br />a. There is knowledge of an emergency incident or threat of an <br />emergency incident; <br />b. Notification can be provided without impeding immediate control of the <br />incident; <br />C. Notification can be provided without impeding employee safety and <br />protection measures (i.e, evacuation, medical response). <br />5. Initial agency notification shall be made verbally and shall provide the <br />following information: <br />a. Name, address and phone number of facility; <br />b. Name and title of facility personnel making notification; <br />C. Location, type and extent of incident (type, extent of damage or injury, <br />type of chemical release, where and how much, etc); <br />d. Time and duration of incident; <br />e. Cause of emergency incident, to the extent known at the time of the <br />initial notification; <br />f. Response/mitigation actions taken. <br />6. Following the initial verbal notification, a written report shall be submitted to <br />the required agencies as soon as possible (reference Table 2) and shall <br />include the following: <br />Draft HMMP <br />March, 2000 HMMP-42 <br />
