a. Name, address and phone number of facility; <br />b. Date, time and duration of incident; <br />C. Location, type and extent of incident (type, extent of damage or injury, <br />type of chemical release, where and how much, etc); <br />d. Cause of emergency incident; <br />e. Response/mitigation/cleanup actions taken; <br />f. Corrective actions initiated and/or scheduled (implementation <br />timetable) to reduce, eliminate and/or prevent the incident from <br />reoccurring. <br />7. When making external notifications the following information shall be <br />documented: <br />a. Agency Name; <br />b. Name of Person Receiving Report; and <br />C. Report Number <br />8. All emergency incident records and reports shall be maintained by the Plant <br />Management for a period of no less than five years. <br />8.8 Agency Coordination <br />1. Coordination will be made with the following agencies: <br />a. County/City Department of Environmental Health: Hazardous <br />Materials Division <br />b. Local Fire and Police Departments <br />C. Office of Emergency Services <br />d. Local Hospital <br />2. Written emergency services agreements (reference the appendix section of <br />this document) shall be sought from the above listed emergency response <br />providers. <br />8.9 EA/CP Distribution <br />1. Copies of the EA/CP shall be distributed as follows: <br />a. County/City Department of Environmental Health: Hazardous <br />Materials Division <br />b. Local Fire and Police Departments <br />C. Office of Emergency Services <br />d. Local Hospital <br />8.10 Amendments to the EA/CP <br />Draft HMMP <br />March, 2000 HMMP-43 <br />
