DRAFT <br />Order will be provided by the testing laboratory and generated with <br />each set of samples. QBR shall insure that the Chain of Custody form <br />will provide, at the minimum, the following information: <br />(1) QBR=s Name and address; <br />(2) Name and phone number of QBR=s contact; <br />(3) Date/time sampled; <br />(4) Analysis required per sample; and <br />(5) Total volume and/or number of containers per sample. <br />X Sample Logs: QBR=s SWPPT shall generate and maintain a Sample <br />Log each time the facility's storm water discharges are sampled for <br />analysis. The following information will be generated and documented <br />by QBR for each sample: <br />(1) Date and time of the sampling; When storm water <br />discharges can not be sampled during the first hour of <br />discharge, an explanation of why sampling did not occur <br />within this 60 minute period must also be provided; <br />(2) Location of the sample; <br />(3) Description of the condition of the sample (i.e., color, odor, <br />cloudiness, pH, etc.); and <br />(4) Name of the QBR personnel responsible for collecting the <br />sample. <br />X Labeling: All sample containers will be pre -labeled by QBR=s <br />designated testing laboratory prior to shipment to the site. Storm <br />water sampler will be responsible for recording the date/time of <br />sampling when the samples are initially collected. At the minimum, <br />each sample container label must include the following: <br />(1) Date and time of sampling; <br />(2) Sample identification; <br />(3) UMP=s name; and <br />(4) Name of person taking the sample. <br />X Storage: Once the samples have been collected, properly logged <br />and labeled, the facility's sampler will be responsible for immediately <br />storing the collected samples at 4° C (39° F) until the collected <br />samples are sent to the designated testing laboratory. At a minimum, <br />samples must be received by the testing laboratory within 48 hours of <br />sampling. All samples released to the laboratory must be <br />accompanied with the completed Chain of Custody form and a copy of <br />the completed Sample Log Sheet. <br />C:\TEMP\SMPREV2.doc Section 2-39 <br />
