MK <br />11/21/97 <br />Received the return receipt for the certified mail, i.e. Cal OSHA has received the referral. <br />Vic Dayco of Cal OSHA called me regarding the referral. He asked me how many employees were there? <br />I told him I did not know, he asked me to estimate. I told him at least 30 employees, he asked me what <br />they did? I told him that they removed old brake shoes, grind it, steamed it and put new shoes on. The <br />brakes were for vehicles and trucks. MK <br />1/98 <br />Placed response in file. MK <br />5/11/98 <br />Per Doug Wilson please printout duplicate permit and waive $10.00 charge. Mary called to confirm <br />address to mail permit. Per Joseph Moreno mail to site address and his atm. MF <br />6/17/98 <br />Received message from Joseph Moreno of QBR regarding drying sludge that contains heavy solid, metal, <br />asbestos and brake fluid, the dry cake will then be taken to land fill. <br />Called Joseph Moreno informing him that he can not do that. That is called treating hazardous waste, <br />tiered permitting is required. MK <br />7/3/98 <br />Placed the report and citation, that Cal OSHA issued to QBR in response to the referral dated 11/20/97, on <br />file. <br />10/1/99 <br />Conducted routine inspection this date.
