The <br /> following records shall be maintained by the project manager at the site: <br /> ' Project Health and Safety Plan, and any subsequent changes; <br /> Project Work Plan,and any subsequent changes; <br /> Site Entry Log; <br /> On-site personnel, their affiliation, and purpose; <br /> Chain-of-Custody Forms <br /> ' Daily Field Log, describing activities and conditions; <br /> Project Safety Inspection Reports; <br /> Tailgate Safety Meeting attendance record; <br /> Worker illness and/or injury reports; <br /> Accident Log. <br /> '. The following records will be kept at the offices of American Geologic Services, Inc. and its <br /> subcontractors: <br /> Accident Logs, OSHA Form 200 or equivalent; <br /> Hazards Materials Training Records; <br /> Medical Surveillance; <br /> Safety Inspection Reports; <br /> • Personal Monitoring Records; <br /> 12.0 CONTINGENCY PLAN, EMERGENCY RESPONSE CONTACTS AND <br /> ' TELEPHONE NUMBERS <br /> In the event of an emergency or accident, the personnel observing the condition shall give three <br /> ' sharp blasts of a vehicle horn, or other prearranged emergency alarm. All nonessential <br /> communication shall immediately cease and the team member shall notify the Project Manager <br /> ' and Health and Safety Officer. Actions to be taken will be dictated by the emergency.
