PUBLIC HtALTH SERVICErS <br /> SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DIVISION "= <br /> Karen Furst, M.D., M.P.H., Health Officer <br /> 304 East Weber Avenue, Third Floor - Stockton, CA 95202 <br /> c4�lFo���P <br /> 2091468-3420 <br /> PUBLIC NOTICE <br /> NOTICE OF FINDINGS REGARDING <br /> SITE MITIGATION PLAN <br /> NOTICE IS HEREBY GIVEN that a Corrective Action Plan has been reviewed by San <br /> Joaquin County Public Health Services, Environmental Health Division (PHS-EHD) for <br /> remediation of a leak associated with underground storage tanks (UST) previously <br /> located at 501 W. Lodi Ave, Lodi (APN 037-310-45) on the northwest corner of <br /> Hutchins Street and Lodi Ave. The Corrective Action Plan is a proposal for mitigation <br /> (soil vapor extraction 1 groundwater air-sparging) of hazardous substances <br /> (petroleum) which have contaminated the site due to a unauthorized release associated <br /> with the USTs which were previously located on the property. <br /> San Joaquin County PHS-EHD, Local UST Oversight Program, has made a tentative <br /> determination that the plan is in compliance with Local, State and Federal laws and <br /> regulations and that the resultant work will not create significant adverse environmental <br /> impacts which exceed the benefits to be gained from cleanup of the contamination. <br /> The corrective action plan is available for public review at PHS-EHD, 304 E. Weber <br /> Street, Third Floor, Stockton, CA., during normal working hours, 8:00 a.m. - 5:00 p.m. If a <br /> written request by any member of the public for a public hearing is not received by <br /> PHS-EHD by 5:00 p.m. Wednesday, September 8, 1999, the tentative decision shall <br /> become final on that date and time. <br /> All requests for public hearings must be filed in writing to PHS-EHD at 304 E. Weber <br /> Ave., Third Floor, Stockton, 95202. Questions regarding the specific site concerns or <br /> procedures should be directed to Michael J.
