Each <br /> label will include: <br /> • Site identification <br /> • Sample Location <br /> • Type of sample <br /> • Date and time of collection <br /> • Name of collector <br /> • Analysis requested <br /> 4.2.7 SAMPLE SHIPMENT AND CHAIN OF CUSTODY <br /> All samples collected for analytical testing will be delivered to an approved analytical laboratory <br /> within 48 hours of collection, or within method holding time. During sample shipment, samples <br /> will be kept on ice or synthetic substitute (e.g. "blue ice")in an insulated container at a <br /> maximum temperature of 4°C. <br /> A chain of custody record,which documents possession of samples from time of collection to <br /> laboratory analysis,will be maintained. Each person who has possession of the samples will <br /> complete applicable sections of the form when samples are received or relinquished and when <br /> sample possession is transferred. <br /> The chain of custody form will be sent in the shipping container along with the samples. The <br /> sample collector will have the receiving personnel of the laboratory sign the chain of custody <br /> form to complete documentation of sample possession from time of collection to delivery to the <br /> laboratory.
