Weber St., Stockton, CA Site Code: PR#506564 <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS- <br /> EHD), has received your request for accounting clarification and offers the following <br /> explanations. <br /> Environmental Assessment sites, such as the above noted addresses, are required to <br /> enter the PHS-EHD Site Mitigation Unit in order to process the papers required to issue <br /> the boring permit to allow the responsible party to conduct the subsurface investigation <br /> your workplan proposed. <br /> The $234 fee was a prepayment to credit your account to cover the workplan review, <br /> the field work, and the final review of the report of findings. It has been the experience <br /> of PHS-EHD that Environmental Assessments that have submitted all of the correct <br /> paperwork and requirements and have a minimum time in the field, expend the balance <br /> of their prepayment. If additional time is needed to correct incomplete submittals, <br /> complete the field work, review the report, or over-time work is necessary, the billing <br /> party identified on the Master File Record Form (MFR) is billed accordingly. <br /> Concerning 201 Main Street, Manteca, it should be noted that multiple phone calls from <br /> Mike Purcell and Tanya Musselwhite of Terranext were received. On February 2, 1997, <br /> March 3, and March 6, 1997, Michael Infurna of the Site Mitigation Unit assisted your <br /> consultants with the requirements of beginning the Environmental Assessment. No <br /> charges were made to this site for the calls or faxes. <br /> On March 9, 1997, the workplan to begin your investigation arrived at PHS-EHD but no <br /> fees were received as required. In order to assist with your scheduling, Mr. Infurna <br /> informed Tanya Musselwhite by telephone that he would start a review of the workplan <br /> on March 10, 1997, with the understanding that the prepayment of$234 would arrive <br /> soon.
