Main St., Manteca, CA Site Code: PR#506554
501 W. Weber St., Stockton, CA Site Code: PR#506564
San Joaquin County Public Health Services, Environmental Health Division (PHS-
EHD), has received your request for accounting clarification and offers the following
explanations.
Environmental Assessment sites, such as the above noted addresses, are required to
enter the PHS-EHD Site Mitigation Unit in order to process the papers required to issue
the boring permit to allow the responsible party to conduct the subsurface investigation
your workplan proposed.
The $234 fee was a prepayment to credit your account to cover the workplan review,
the field work, and the final review of the report of findings. It has been the experience
of PHS-EHD that Environmental Assessments that have submitted all of the correct
paperwork and requirements and have a minimum time in the field, expend the balance
of their prepayment. If additional time is needed to correct incomplete submittals,
complete the field work, review the report, or over-time work is necessary, the billing
party identified on the Master File Record Form (MFR) is billed accordingly.
Concerning 201 Main Street, Manteca, it should be noted that multiple phone calls from
Mike Purcell and Tanya Musselwhite of Terranext were received. On February 2, 1997,
March 3, and March 6, 1997, Michael Infurna of the Site Mitigation Unit assisted your
consultants with the requirements of beginning the Environmental Assessment. No
charges were made to this site for the calls or faxes.
On March 9, 1997, the workplan to begin your investigation arrived at PHS-EHD but no
fees were received as required. In order to assist with your scheduling, Mr. Infurna
informed Tanya Musselwhite by telephone that he would start a review of the workplan
on March 10, 1997, with the understanding that the prepayment of$234 would arrive
soon. No time was charged to your account for this date either.
A Division of San Joaquin County Health Care Services
