REPORT CONTENT <br /> Each report should be a complete entity. If the report lacks data as described <br /> below, such as pagination, it should be returned to the tank owner for completion <br /> prior to resubmittal for regulatory review. <br /> PRELIMINARY PEPOP.T <br /> Name and address of site, tank owner, and contact person, If different <br /> Map showing site location relative to nearby landmarks (North is up) <br /> Sketch of the site showing nearby streets and properties <br /> Layout of all tanks and piping systems properly labeled for content <br /> Results of tank tests or Initial site work done <br /> Estimates of liquid quantity and composition leaked Into the environment <br /> Local topography, geology, nearby streams, depth to ground water <br /> Proximity of private or municipal wells and Irrigation wells <br /> Location of nearby septic tanks, underground public utility lines <br /> Distance to adjacent buildings and their water and sewer lines <br /> Proposed sampling locations and methodology <br /> Based upon this information, the LIA and Regional Board probably could accept the <br /> initial proposal to begin work. Once the driflinq hays provided data, the consultant <br /> could then follow up the Preliminary Report ;vith a letter describing the findings <br /> and any additional borings on the moditied site reap needed to define the problem.'We could review this proposal quicHy and either approve verbally, in writing, or <br /> discuss at a meeting with the tank owner and consultant.
