(Stratus) has prepared this Well Destruction Report, on behalf
of Moller Investment Group, Inc. (MIGI), for former USA Station No. 65 (the site),
located at 2500 West Lodi Avenue, Lodi, California (see Figure I and Figure 2). At the
verbal request of San Joaquin County Environmental Health Department (SJCEHD),
Stratus prepared and submitted a document titled Well Destruction Work Plan on
September 22, 2008. The purpose of the Work Plan was to present a scope of work
outlining work activities necessary to destroy all existing monitoring and remediation
wells located at the subject site, prior to closure of the site's environmental case. At the
recommendation of SJCEHD personnel, the California Regional Water Quality Control
Board (RWQCB) indicated, in a letter dated December 22, 2008, that closure of the site's
environmental case would be granted once all of the wells at the site have been properly
destroyed. Stratus recently oversaw destruction of these wells; information pertaining to
the completion of this work is documented in this report.
WELL DESTRUCTIONS
Prior to initiating work activities, a well destruction permit was obtained from SJCEHD;
a copy of this permit is provided in Appendix A. The work areas were marked for
Underground Service Alert (USA) clearance 48 hours in advance and SJCEHD, MIGI,
the operator of the site service station (Tesoro), and USA were notified in advance of
completion of work activities. The former well locations are presented in Figure 2. All
work was completed under the direction of a State of California Professional Geologist.
A C-57 licensed drilling contractor (Woodward Drilling Inc., C-57 #710079) performed
the well destruction activities, in the presence of a Stratus representative, between
KAUSA Gasoline100651Rcportslusa65-well destruction report.doc
