(Stratus) has prepared this Well Destruction Report, on behalf <br /> of Moller Investment Group, Inc. (MIGI), for former USA Station No. 65 (the site), <br /> located at 2500 West Lodi Avenue, Lodi, California (see Figure I and Figure 2). At the <br /> verbal request of San Joaquin County Environmental Health Department (SJCEHD), <br /> Stratus prepared and submitted a document titled Well Destruction Work Plan on <br /> September 22, 2008. The purpose of the Work Plan was to present a scope of work <br /> outlining work activities necessary to destroy all existing monitoring and remediation <br /> wells located at the subject site, prior to closure of the site's environmental case. At the <br /> recommendation of SJCEHD personnel, the California Regional Water Quality Control <br /> Board (RWQCB) indicated, in a letter dated December 22, 2008, that closure of the site's <br /> environmental case would be granted once all of the wells at the site have been properly <br /> destroyed. Stratus recently oversaw destruction of these wells; information pertaining to <br /> the completion of this work is documented in this report. <br /> WELL DESTRUCTIONS <br /> Prior to initiating work activities, a well destruction permit was obtained from SJCEHD; <br /> a copy of this permit is provided in Appendix A. The work areas were marked for <br /> Underground Service Alert (USA) clearance 48 hours in advance and SJCEHD, MIGI, <br /> the operator of the site service station (Tesoro), and USA were notified in advance of <br /> completion of work activities. The former well locations are presented in Figure 2. All <br /> work was completed under the direction of a State of California Professional Geologist. <br /> A C-57 licensed drilling contractor (Woodward Drilling Inc., C-57 #710079) performed <br /> the well destruction activities, in the presence of a Stratus representative, between <br /> KAUSA Gasoline100651Rcportslusa65-well destruction report.doc <br />
