28 February 2012 <br /> AGE-NC Project No. 98-0497 <br /> Page 2of3 <br /> activities at the site in October and November 2006, dissolved concentrations in well MW-1 <br /> were significantly reduced. Petroleum hydrocarbons were not reported (non detect)above <br /> laboratory detection limits in any ground water monitoring well samples since February <br /> 2007. Analytical results of ground water samples are summarized in Table 3. <br /> As a result of the documented non detect wells at the site, AGE proposes to destroy wells <br /> MW-1 through MW-6 by pressure grouting techniques. All wells will be destroyed in <br /> accordance with the EHD Standards For Well Construction and Destruction. <br /> PROCEDURES <br /> As required, a health and safety plan will be prepared in accordance with Occupational <br /> Safety and Health Guidance Manual for Hazardous Waste Site Activities(National Institute <br /> for Occupational Safety and Health Administ ation, U.S. Coast Guard and U.S. <br /> Environmental Protection Agency, 1985). Prior to drilling activities, each proposed drilling <br /> location will be clearly marked and an utility clearance obtained through Underground <br /> Service Alert. <br /> Prior to destruction activities, wells will be check d for total depth and integrity using a <br /> Solinst water level meter. Any obstructions found in the well will be removed. <br /> Well boxes and caps will be re lived to expose the well casings of wells MW-1 through yl <br /> MW-6. The well heads will b fitted with a press re tight adapter connected to a hose <br /> extending from a grout mixe . A standard grout mixture will,be pumped into the well casing <br /> under approximately A pounds per square inch (psi)of pressure. The well will then be <br /> over-drilled to approximately 3 feet below surface grade (bsg) using 8.25-inch diameter°_ <br /> hollow stem augers. The top 2.5 feet of the well c sing will then be cut and removed and ab"�. , <br /> grout will be used to fill the boring to approximately 0.5 feet above the top of the well c 4`r <br /> casing. The remaining borehole will be backfilled to surface grade with neat cement and '�t. <br /> covered with concrete or asphalt or rock, dependi g on the surrounding surface material. -1-ham <br /> Soil cuttings generated during drilling activities and rinseate will be containerized in <br /> properly labeled Department of Transportation ( .O.T. -a PProved 55-gallon drums or a <br /> landfill disposal container, and stored on-site for di 3posal, in an area lacking public access. <br /> Upon completion of field work and disposal of soil-cuttings, a Well Destruction Reportort <br /> describing well destruction activities and proced res, including documentation of waste <br /> disposal from well destruction and other activities, will be prepared and submitted to the <br /> EHD. <br /> Advanced CeoEnvironmental,Inc. <br />
