Main Street, Stockton CA. <br /> Page 3 <br /> A workplan regarding assessment for the above referenced site submitted by Pacific <br /> Environmental Group Inc. (PEG) was reviewed by San Joaquin County Public Health Services <br /> Environmental Health Division (PHS/EHD). PHS/EHD provided comments regarding <br /> substantial deficiencies noted in that plan after many conversations with consultants and <br /> responsible parties at this site and subsequently faxed those comments to Chevron on April 18, <br /> 1996. Of highest priority was the subject of closing Chevrons abandoned underground waste <br /> oil storage tank and any other potential sources. After several telephone conversations with the <br /> property owner and Chevron representatives including PEG staff, Brett Hunter of Chevron was <br /> verbally notified again on August 5, 1996 that tank and related appurtenances closure was the <br /> primary and highest priority item on the agenda for this site. <br /> On August 5, 1996 Chevron representative Brett Hunter was notified of pending enforcement <br /> actions with regard to the above referenced site for failure to comply with a multitude of <br /> potential violations. Following this conversation the RP (Sherman Hinaman) was also notified <br /> of this potential enforcement action. On August 7, 1996 Brett Hunter spoke with the supervisor <br /> of the PHS/EHD Site Mitigation Unit providing a verbal commitment to remove the tank, other <br /> potential sources and also requested written comments on the deficiencies of the workplan <br /> submitted previously. The following comments are consistent with those directives and <br /> suggestions found on the faxed version dated April 18, 1996. <br /> A Ground Penetrating Radar (GPR) assessment of the site should be conducted at the site prior <br /> to workplan approval in order to determine the presence of potential sources left by Chevron as <br /> a result of incomplete demolition and closure as suggested by reviewing the <br /> demolition/abandonment report generated by Chevron engineers in 1976 and the findings of the <br /> MARK Environmental Assessment report. The results of this work shall submitted and be <br /> incorporated in the workplan amendment. <br /> Immediately obtain/submit all necessary permits and applications required for the removal <br /> of all known or suspected tanks, piping and potential sources or debris which may preclude <br /> the full assessment and characterization at this site. <br /> Submit a Health and Safety Plan (HSP) and contact this office for appointments to witness all <br /> site work related to this assessment 48 hours in advance by contacting Ronald Rowe, Sr. REHS, <br /> at (209) 468-0342. All site work to be witnessed by the contact at PHS/EHD listed above: <br /> In the event that wells are justified and placed at this site they shall be designed so as to provide <br /> full vertical and lateral definition. <br />
