These concerns shall be addressed in a Preliminary Report (see Tri Regional <br /> Board Recommendations for guidance) and an Acknowledgment Letter must be <br /> submitted by the Responsible Party/Property Owner prior to further field N� <br /> investigative work. <br /> All proposals and reports submitted must be accompanied by a Letter of <br /> Acknowledgement from the responsible party(ies) which states, at a minimum, <br /> the following: <br /> I(We) declare, under penalty of perjury, that the information <br /> and/or recommendations contained in the attached proposal or <br /> report is/are true and correct, and that all work and reports <br /> which required geologic or engineering evaluations and/or <br /> judgments have been performed under the direction of an <br /> appropriately registered or certified professional . - <br /> The attached proposal or report utilizes the "Regional Board Staff <br /> Recommendations For Initial Evaluation and Investigation of <br /> Underground Tanks" and "Appendix A" reporting format. The LUFT <br /> Manual has also been utilized as a guidance document. <br /> Furthermore, I(We) agree to submit, on the date noted, the <br /> following on the site(s) in question: <br /> i . . . . . . . ..Preliminary Report" by (date) to the <br /> San Joaquin County PHS/EHD and the Central Valley Regional Water Quality <br /> Control Board (CVRWQCB). <br /> Your acknowledgement of the above is respectfully requested. In addition, the <br /> individual or individuals authorizing the continued investigation shall <br /> provide identification with respect to their title of authority. <br /> Include in all proposals and reports a site specific Health and Safety Plan <br /> describing personal protection for all field personnel (see 29 CFR 1910.120 of <br /> OSHA regulations). <br /> This protection should include the use of personal protective equipment <br /> such as coveralls, gloves, steel toe and shank shoes or boots (leather <br /> or chemical resistant material) safety glasses, and hard hats. <br /> Precautions should be taken to prevent excessive inhalation of/or <br /> exposure to contaminated soils. <br /> The San Joaquin County PHS/EHD will, continue to review proposals for the <br /> purpose of compliance to local ordinance requirements for permits for certain <br /> activities such as borings, monitoring wells/piezometers, vapor probe surveys, <br /> extraction systems, etc. <br />
