The <br /> groundwater analytical data table in the report failed to show analytical results for all of the <br /> oxygenates tested. Groundwater monitoring analytical data with corresponding sampling event <br /> dates was not presented in tabulated form. Groundwater quarterly monitoring analytical data <br /> must be provided to PHS/EHD in all reports in tabulated form showing every contaminant <br /> analyzed and chronological dates of sampling events. Data tables must be updated in each report <br /> to show the range of contaminant concentrations starting with the baseline sampling event to the <br /> most recent sampling event. <br /> The report did not contain a geologic description of the site. A complete geologic description of <br /> the site must be provided to PHS/EHD in every report. It is crucial to identify all potential <br /> environmental and public health risks, lithology,preferential pathways, and receptors in order to <br /> fully characterize the site, evaluate the contaminant threat, and determine remediation methods. <br /> All future work plans and reports for this site must contain details listed in Appendix A. <br /> Information as described above must be submitted to PHS/EHD to complete the report. Future <br /> reports that are not completed according to Appendix A will be returned to the sender. <br /> A work plan to propose contaminated soil and groundwater remediation determine the lateral <br /> and vertical extent of groundwater contamination, and provide the information described above <br /> must be submitted to PHS/EHD by January 31, 2001.
