Main St. <br /> Stockton, CA 95212 <br /> San Joaquin County Public Health Services Environmental Health Division (PHS/EHD) <br /> requested addendum information for an Additional Characterization Work Plan (Work Plan) <br /> dated April 15, 2000 prepared by John P. Cummings &Associates for the above referenced site. <br /> The addendum information has not been received by this office. <br /> Quarterly Monitoring (QM) of the groundwater at the site was one of the required addendum <br /> components for the Work Plan. QM was attempted at the site on May 22, 2000. Prior to the <br /> collection of monitoring well samples, parameter measurements for electrical conductivity, <br /> temperature and pH were not obtained. Analytical results for this monitoring event can not be <br /> recognized by PHS/EHD as representative of the water quality. Also, QM Reports (QMRs)have <br /> not been received by PHS/EHD for this site since October 12, 1999. Please refer to the Tri- <br /> Regional Board Staff Recommendations for Preliminary Investigation and Evaluation of <br /> Underground Tank Sites, Appendix A, page A-3, under Quarterly Status, for guidance on <br /> conducting QM and submitting QMRs. <br /> Analytical results from soil and groundwater samples collected at this site evidence significant <br /> TPHg, BETX, and high levels of MTBE. Due to the high contaminant concentrations in soil and <br /> groundwater, appropriate characterization of the contamination at this site should occur as soon <br /> as possible. Interim mitigation of MTBE contamination in the groundwater may be necessary to <br /> control potential off site migration of the plume. The above referenced Work Plan is approved <br /> by PHS/EHD with the conditions listed below: <br /> 1. All work outlined in the Work Plan must be completed within 30 working days of the <br /> date of this letter. <br /> 2. Proper QM must be completed within 30 working days of the date of this letter. <br /> 3. A well permit application and $89 permit fee should be submitted to PHS/EHD.
