Main St. <br /> Stockton,CA 95212 <br /> As the responsible party,you were notified on January 31,2000 that a Work Plan and Well Survey Report <br /> should be submitted to San Joaquin County Public Health Services Environmental Health Division <br /> (PHS/EHD)within 14 days of the date of that letter. A meeting was requested by the responsible party and <br /> took place at the PHS/EHD office on February 16,2000. During this meeting the responsible party <br /> requested an extension to allow for more time to submit the Work Plan and Well Survey. <br /> PHS/EHD staff discussed this site with State Water Resources Control Board Cleanup Fund staff. <br /> PHS/EHD was informed that the corrective action costs as submitted were not eligible. You as the <br /> responsible party must resubmit the reimbursement claim to meet their specifications. <br /> A Work Plan and Well Survey should be submitted to PHS/EHD by April 25.2000. <br /> The PHS/EHD November 3, 1999 and January 31,2000 letters also directed that quarterly groundwater <br /> monitoring be continued at the site. No Quarterly Monitoring Report(QMR)has been received since the <br /> September 24, 1999 QMR.The site is currently out of compliance with PHS/EHD directives to perform <br /> QM and submit all QMRs. Provide PHS/EHD with the QMR for the last quarter of 1999.
