Main St. <br /> Stockton, CA 95212 <br /> As the responsible party, you were notified on November 3, 1999 that a Work Plan for <br /> further investigation of contamination at this site and a Well Survey Report should be <br /> submitted to San Joaquin County Public Health Services Environmental Health Division <br /> (PHS/EHD) by January 5, 2000. This office has not received the Work Plan and Well <br /> Survey Report. <br /> The Work Plan and Well Survey Report must be submitted to PHS/EHD within 14 days <br /> of the date of this letter. If a Work Plan is not submitted to this office within 14 days of <br /> the date of this letter, fonnal enforcement action by the San Joaquin County District <br /> Attorneys Office may be requested and any reimbursement of corrective action costs by <br /> the State Water Resources Control Board (SWRCB), Clean Up Fund may be jeopardized. <br /> The PHS/EHD November 3, 1999 letter also directed that quarterly groundwater <br /> monitoring be continued at the site. No Quarterly Monitoring Report (QMR) has been <br /> received since the September 24, 1999 QMR. Provide PHS/EHD with the QMR for the <br /> last quarter of 1999.
