ADDITIONAL ASSESSMEORKPLAN <br /> JAM"SERVICE, STOCKTON <br /> DECEMBER 2001 <br /> An encroachment permit will be required from San Joaquin County for the well to be placed in East <br /> Main and right-of-entry agreements will be required for the two wells to be placed on private <br /> property <br /> Utility Survey <br /> The Underground Services Alert(USA)network will be contacted at least 48 hours before drilling <br /> operations begin to locate underground utilities on the site and adjacent public right-of-ways The <br /> first 5-feet of each boring will be hand augured as a further precaution to avoid underground <br /> utilities <br /> Health and Safety Plan <br /> A site specific HSP will be developed by GHH to minimize the potential for accidents or potentially <br /> dangerous exposure to chemicals,as well as to identify the nearest emergency medical facility A <br /> draft copy of the HSP is included in Appendix A <br /> PROPOSED FIELD ACTIVITIES <br /> Monitoring Well Installation <br /> Based on the present distribution of petroleum hydrocarbons and MTBE in on-site groundwater <br /> monitoring wells and the groundwater flow patterns determined from quarterly groundwater <br /> monitoring at the site, GHH proposes to install three new off-site groundwater monitoring wells <br /> (MW-6 through MW-8)at the locations shown on Figure 4 The new wells will be used to monitor <br /> the vertical and horizontal extent of impacted groundwater off-site The new wells will also provide <br /> Rather information on the gradient and direction of groundwater flow Drilling and well installation <br /> activities will be reviewed by a California Registered Geologist or Licensed Civil Engineer <br /> Dnlling will be performed by hollow-stem auger drilling techniques using a truck-mounted drill rig <br /> equipped with 8-inch outside diameter hollow-stem augers Prior to drilling, augers and associated <br /> drilling and soil sampling equipment will be cleaned with a high pressure hot water washer To <br /> minimize the possibility of cross contamination, the sampling equipment and brass liners will be <br /> washed and rinsed with distilled water before collecting each soil sample Drilling and sampling <br /> will be performed according to ASTM Method D 1452-80, Soil Investigation and Sampling by <br /> Auger Borings as described herein <br /> Soil samples will be collected at 5-foot intervals for classification by the Unified Soil Classification <br /> System(USCS), field screening and for possible chemical analysis Soil samples will be collected <br /> by placing the augers above the desired sampling depth and then driving a modified California split- <br /> spoon sampler lined with three brass tubes into the undisturbed soil <br /> 4 <br />
