C5
custcA- y form and returned to my office in Fremont, California. I then placed the samples
in my refrigerator. I arrived at my office approximately 2:34 P. M., which was too late tc
ship the samples so as to arrive in Sacramento that evening in time to be picked up by
the laboratory personnel.
4n Wednesday morning, December 22, 1993, 1 removed all of the samples from the
refrigerator and packed them in an insulated packing box with a large block of ice. All of
the Chain-of-custody forms from butt 64implinq events were stapled together and the last
page was signed, for my release. The forms were placed in a waterproof bag and sealed
inside the box with the samples. They were then transported by me to the Greyhound Bus
Terminal in Framont with requested "next bus out" service to Sacramento with instructions
to call Sparger Technology upon arrival.
Should you have any questions or which to discuss this matter in more detail, I would be
happy to meet with you at your convenience. If the Environmental Health bivision has
pre-axisting writter procedures that are contrary to my handling of the samples, I would
appreciate it if you would send me a copy as soon as possible.
erely,
tiin N.ALF
John N.
