Request for Proposal <br /> August 31, 1988 <br /> Page 2 <br /> INITIAL ASSESSMENT OF SITE CONTAMINATION FROM <br /> LEAKING UNDERGROUND FUEL TANKS AT THREE LOCATIONS <br /> 4. Prepare a "Final Remedial Plan" for implementation on <br /> a time schedule. <br /> All reports shall be in a form acceptable to the San Joaquin <br /> Health District, the Regional Water Quality Control Board and <br /> the City of Stockton. They shall follow the format and <br /> contain the information as outlined in the attached Regional <br /> Water Quality Control Board, Central Valley Region memorandum <br /> entitled "Consultant Reports for Assessing Ground Water <br /> Problems at Sites Contaminated by Leaking Underground Storage <br /> Tanks (LUSTS) " dated March 24, 1987. <br /> PROPOSAL EVALUATION <br /> Proposals will be evaluated based on completeness, conformance <br /> to Regional Water Quality Control Board regulations and <br /> costs. Due to the nature of the project accurate costing may <br /> not be possible but present a budget that realistically <br /> addresses the cost of completing the evaluation and <br /> development of a remedial plan. <br /> As a minimum the proposal should include, for each site, costs <br /> for: <br /> Installation of three (3) monitoring wells to <br /> determine groundwater gradient. Soil sampling and <br /> testing at five (5) foot intervals. Lab analysis for <br /> one (1) water sample and two (2) soil samples from <br /> each monitoring well. <br /> Installation of two (2) soil borings with sampling and <br /> testing at five (5) foot intervals. Lab analysis for <br /> two (2) samples from each boring. <br /> The proposal must list the personnel that will work on the <br /> project and their qualifications. A project manager must be <br /> designated. This person will be named in the actual contract <br /> and cannot be changed without prior written approval from the <br /> City once the contract is executed. <br /> The City requires the contractor, as a minimum, to pay <br /> prevailing wage rates for each classification employed in the <br /> conduct of this project. Prevailing wages are established for <br /> this region by the Prevailing Wage Board. Their determination <br /> shall be final. The successful contractor will be required to <br /> provide a Performance Bond, ensuring completion of the work, <br />
