Consent for inspection, sampling, and photographs was granted by Ms. Elisa Moberly <br />of San Joaquin County Public Works. The inspection was conducted by Mr. Muniappa Naidu and Ms. <br />Elena Manzo, of the San Joaquin County Environmental Health Department (EHD). During the <br />inspection, the EHD inspectors were accompanied by Ms. Elisa Moberly and Ms. Maelee Janvrin (011ie), <br />the PHHWF supervisor. Inspection was performed on June 27, 2014 and June 30, 2014; the checklist of <br />findings was provided at the time of inspection. The full inspection report was provided to PHHWF on <br />August 7, 2014. <br />Class II Violations: 111, 301, 401, 605, 608, 804, 9999 <br />111. The contingency plan was not available on site at the time of inspection. A copy of the contingency <br />plan and all revisions to the plan shall be maintained at the facility at all times. A contingency plan shall <br />include all of the following: <br />-actions facility personnel shall take in response to fires, explosions, and unplanned release to air, <br />soil, or surface water <br />-arrangements agreed to by local police departments, fire departments, hospitals, cleanup contractors, <br />and state & local emergency response teams <br />-names, addresses & phone numbers (office & home) of the Emergency Coordinator and any <br />alternates <br />-list of all emergency equipment at the facility (fire, spill, communications/alarms, decontamination), <br />including location physical description, and a brief outline of its capabilities <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE <br />Inspecroc r/-�//������ <br />Dee M� <br />T <br />Title: <br />• IOr'y <br />G� r. <br />EHD 22-02 Rev 04/19/12 Page 1 ( HW INSPECTION REPORT <br />
