SUMMARY OF VIOLATIONS CLASS I, CLASS II, or MINOR -Notice to Comply) Onsite to conduct a routine hazardous waste inspection at the Permanent Household Hazardous Waste Facility (PHHWF). Consent for inspection, sampling, and photographs was granted by Ms. Elisa Moberly of San Joaquin County Public Works. The inspection was conducted by Mr. Muniappa Naidu and Ms. Elena Manzo, of the San Joaquin County Environmental Health Department (EHD). During the inspection, the EHD inspectors were accompanied by Ms. Elisa Moberly and Ms. Maelee Janvrin (011ie), the PHHWF supervisor. Inspection was performed on June 27, 2014 and June 30, 2014; the checklist of findings was provided at the time of inspection. The full inspection report was provided to PHHWF on August 7, 2014. Class II Violations: 111, 301, 401, 605, 608, 804, 9999 111. The contingency plan was not available on site at the time of inspection. A copy of the contingency plan and all revisions to the plan shall be maintained at the facility at all times. A contingency plan shall include all of the following: -actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, or surface water -arrangements agreed to by local police departments, fire departments, hospitals, cleanup contractors, and state & local emergency response teams -names, addresses & phone numbers (office & home) of the Emergency Coordinator and any alternates -list of all emergency equipment at the facility (fire, spill, communications/alarms, decontamination), including location physical description, and a brief outline of its capabilities ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
