Ms. Elena Manzo <br />STATEMENT OF CORRECTIVE ACTIONS — SJC HHW FACILITY <br />1. Manifests for outgoing shipments are faxed to clerical support personnel to <br />enter on a tracking spreadsheet. <br />2. Solid Waste personnel will inquire, by email, with our HHW Contractor, PSC, as <br />to the status of any manifests for which a signed copy from the designated <br />facility has not been received after 30 days. <br />3. PSC (the County's HHW contractor) has agreed to respond within 10 days as <br />to the status of those shipments. <br />4. Solid Waste personnel will submit an exception report to DTSC for any signed <br />manifest still not received by the 45`h day. <br />Pg. 7, item 301.) County Solid Waste recognizes the seriousness of resolving this issue. Because <br />County staff does not have the expertise to address the problems with the HHW <br />floors and containment sumps, a Request for Proposals was already underway at <br />the time of inspection in order to gain assistance from a consulting firm with such <br />expertise. This RFP, which was in process, has been expedited. <br />At this time, the schedule for this work is as follows: <br />Release of the Request for Proposal July 26, 2013 <br />Deadline for delivery of SOQ August 16, 2013, 4:00 p.m. <br />Notification of Consultant(s) August 30, 2013 <br />Contract signed and Notice to Proceed Late September 2013 <br />Solid Waste personnel will direct this consultant to address the condition of the <br />Household Hazardous Waste facility floors immediately upon signing the <br />contract. <br />Pg. 8, item 301.) Non-empty/leaking containers (trash receptacle) — The HHW facility has a <br />standard procedure for the draining of automotive fluid containers. The container <br />is placed upside down on a drain rack while it is draining into the appropriate <br />reservoir. Once dripping has stopped, absorbent (sand) is placed into the <br />container; lastly, the container is discarded in the trash receptacle. A photo of <br />the drain rack over the receptacle is attached. Employees are all familiar with <br />this process. <br />Regarding the finding of a non -empty container on 6/26/13, the following was <br />discovered: On or about 6/29/13, HHW Supervisor, 011ie Janvrin, reviewed the <br />security video in order to determine who placed the non -empty automotive fluid <br />containers in the trash receptacle. She found that someone had entered the <br />facility while it was closed and placed automotive fluid containers in the trash <br />receptacle. When Ms. Janvrin attempted to recall the video on 7/13/13 in <br />preparation for a meeting with me on 7/16/13, the video clip was no longer <br />accessible. Solid Waste personnel have reviewed the system and determined <br />that it is currently keeping one week's worth of video. <br />As a preventive measure, the County is in the process of consulting with fencing <br />companies regarding the installation of security fencing as a barrier to individuals <br />which may try to enter the premises during the hours that the facility is closed. <br />Page 2 of 3 <br />
