Within 30 days of each shipment of hazardous waste, the generator shall submit to the Department a legible copy of each manifest (CCR §66262.23(a)(4)). The generator manifest copy shall be mailed to: DTSC Generator Manifest P.O. Box 400 Sacramento, CA 95812-0400 PHHWF to submit proof by August 12, 2013, that above manifests were sent to the Department within the requisite period of time. If such proof cannot be provided, PHHWF shall send a written request to the Department inquiring as to the status of said manifests. Legible copies of the aforementioned manifests must be included with the written request. The inquiry letter shall be mailed to: Department of Toxic Substances Control GISS, P.O. Box 806 Sacramento CA 95812-0806 PHHWF to submit proof by August 12, 2013, that said written request was submitted to the Department, as required above. 122. 123. 124. The PHHWF failed to keep signed copies of manifests from the designated facility for 3 years, failed to determine the status of hazardous waste when copies of manifests signed by the designation facility were not received within 35 days, and failed to file an exception report when copies manifests signed by designation facility were not received within 45 days.
