If the generator did not receive a copy <br />of the manifest with the handwritten signature of the owner or operator of the facility to which the <br />generator's waste was submitted within 30 days of the date the waste was accepted by the initial <br />transporter, the generator shall submit an exception report containing a legible copy of missing manifest <br />and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the <br />results of those efforts. <br />Exception reports shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Immediately locate copies of missing signed off manifests or prepare and submit an exception report to <br />DTSC. By August 12, 2013, submit to the EHD copies of the signed off manifests or a copy of the <br />exception report(s) submitted to DTSC. <br />301. During the inspection it was noted that the floor of the secondary containment sump in the drum <br />storage area is cracked and peeling. According to the PHHWF supervisor, 011ie, when it rains water <br />comes into the building and often they work in six inches of water. Also, 011ie stated that on at least one <br />occasion in the past they pumped fifteen 55 -gallon drums of runoff water from inside of the secondary <br />containment sump. The containment area of the PHHWF shall be designed so that a base shall underlie <br />the containers. The base shall be free of cracks or gaps and be sufficiently impervious to contain leaks, <br />spills, and accumulated precipitation until the collected material is detected and removed. Run-on into <br />the containment system shall be prevented unless the collection system has sufficient excess capacity in <br />addition to that required to contain precipitation from at least a 24-hour, 25 -year storm plus 10% of <br />aggregate volume of all containers of the volume of largest container whichever is greater. Also, the <br />facility shall be maintained and operated to minimize the possibility of a fire, explosion, or release of <br />hazardous waste to air, soil, or surface water which could threaten human health or the environment. <br />By August 12, 2013 submit a report describing how the floor of the containment sump shall be <br />resurfaced; the report shall also include an assessment signed by a professional engineer evaluating the <br />capacity of the existing collection system.
