• 13 0 ACCIDENT REPORTING <br /> Accident reporting is an individual responsibility All individuals who are injured must <br /> ' immediately report the accident, however minor, to their supervisors Supervisors will obtain <br /> all pertinent information so that proper forms can be completed in the required number and <br /> forwarded to the Project Safety Officer within two working days <br /> Any accident involving bodily injury that requires medical treatment beyond that which <br /> can be adequately and safely rendered on the project site must be reported to The MARK <br /> Group, Engineers & GeoIogists, Inc (MARK) respective Managing Principal and Corporate <br /> Health and Safety Officer within 24 hours <br /> The Project Safety Officer will keep an accident log at the job site Upon completion <br /> of the project, this log will be made part of MARK's central personnel files <br /> Any accident, whether or not injury to personnel is involved, shall be investigated by <br /> the Project Safety Officer and measures promptly taken to avoid recurrence If the accident <br /> is of a character that, except for circumstances, it may have been immediately dangerous to <br /> life or health (IDLH), it shall be reported to MARK's respective Managing Principal and <br /> Corporate Health and Safety Officer within 24 hours Accident reports shall be submitted <br /> to federal and state authorities in compliance with applicable regulations and to the client <br /> or other interested parties according to the conditions of the contract <br /> • <br /> 13-1 REV 4192-EGI <br /> t���i <br />
