Suggestion Report (see attached) <br />' This form should be completed each time a potential hazard is noticed in the work <br /> environment or lob site Since it is MARK'S policy to promote and provide a safe and <br /> healthy work environment, employees are encouraged to be aware and to report any <br /> 1 problems that they observe There will be no retaliation or negative feelings fostered toward <br /> any employee who reports potentially dangerous or hazardous problems Employees should <br /> feel free to report problems without fear of reprisal It is for the safety of all employees that <br /> these problems be itemized so that corrections may be made <br /> Employee Training Report (see attached) <br /> This form should be used to itemize (generally and specifically) those areas in which <br /> new employees have been trained Any time a lob change is made for an existing employee, <br /> 10 this form should be used to itemize the new training that has been provided to the employee <br /> It is MARK'S policy to promote and provide a safe and healthy work environment It is with <br /> this in mind that we require all employees to be aware of our safety codes and standards <br /> Personnel should be able to recognize hazards and to take the necessary steps to eliminate <br /> these hazards A supervisor should provide general training, as well as any specific training <br /> relating to the particular project, lob or position that the employee is assigned to Safety <br /> issues and hazards relating to specific job assignments should also be discussed at this time <br /> General Guidelines <br /> Field Work <br /> Please consult MARK'S Master Health and Safety Plans, Revised April 1992, for <br /> Igeneral and specific information on health and safety compliance in the work site <br /> H&S-PLAN ADD Page 3 REVISED APRIL 1992 <br />
