r <br /> ■ PLEASANT HILL <br />' Contra Costa Occupational Medicine Associates <br /> 1100 Contra Costa Boulevard <br /> Concord, CA 94523 <br />' Telephone 510/825-3400 <br /> Employee medical records are retained in confidence for 30 years by the corporation <br /> The MARK medical surveillance program includes a medical examination consisting of a <br /> medical and work history, physical examination that includes vital signs and an evaluation <br /> of all major organ systems including skin, audiogram, vision screening, chest x-ray, composite <br /> blood chemistry (CBC), SMAC 24 biochemical profile (additional blood chemistry), <br /> urinalysis to evaluate kidney/liver functions, and pulmonary function tests, which include <br /> forced expiratory capacity at 1 second (FEVI) and a forced vital capacity (FVC) The entire <br /> rexamination is repeated annually, with the exception of the chest x-ray, or upon employee <br /> termination, whichever occurs first In addition, if a worker has been exposed to a known <br /> or suspected release, he/she will undergo immediate additional testing at the direction of the <br /> MARK occupational health physician <br /> All subcontractor personnel involved in operations where they may be exposed to <br /> hazardous or toxic materials shall have successfully completed screening medical <br /> examinations similar to those required for MARK personnel This medical surveillance shall <br /> be the responsibility of the subcontractor <br /> 52 Emergeng Medical Treatment <br /> In the case of an accident involving personal injury or other medical emergency, the <br />' <br /> following procedures apply <br /> ■ For emergencies requiring firefighting, police, or ambulance (medic) services, <br /> . procedures for contacting the emergency services will be established prior to <br /> project startup Appropriate telephone numbers shall be posted <br /> r <br /> 5-2 REV 4/92-EGI <br />
