■ Hospital locations and telephone numbers shall be identified prior to project <br /> startup They will be posted on-site <br /> ■ Adequate numbers of first aid kits (at least one, regardless of project size) shall <br /> be available at known locations on the job site and shall be readily accessible to <br /> all employees <br /> ■ If an injury or illness clearly requires only first aid procedures, treatment can be <br /> limited to that level All incidents not obviously limited to first aid treatment <br /> levels require the activation of the appropriate resources to provide more <br /> definitive medical care <br /> ■ Any injury or illness will require the completion of a MARK employee injury <br /> report This report when completed by the employee's supervisor, shall be <br /> forwarded to the Managing Principal or the Corporate Health and Safety Officer <br /> as soon as possible <br /> ■ In addition to the above requirements, any injury/illness not limited to first aid <br /> care will require that the immediate supervisor contact the Project Safety Officer <br /> This will allow coordination of internal resources to advise the treating physician <br /> of the circumstances of the injury and of any circumstances related to the case <br /> It will also permit a timely accident investigation to determine underlying causes <br /> so that appropriate corrective and preventive steps may be taken to prevent <br /> recurrence <br /> ■ If any injury or illness is the result of an exposure to a chemical or a hazardous <br /> waste, a supervisor shall promptly initiate the steps necessary to identify the <br /> chemical(s) involved Such information shall be made available to the treating <br /> physician and MARK management <br /> 53 Symptomatic Monitoring <br /> On projects where there is risk of exposure to hazardous or toxic materials, all <br /> personnel shall be instructed to monitor themselves and their co-workers for any sign of toxic <br /> exposure If any of the following symptoms are detected, they shall be immediately reported <br /> to the worker's supervisor <br /> ■ changes in complexion and skin discoloration, <br /> ■ changes in coordination, <br /> ■ changes in demeanor, <br /> ■ excessive salivation and abnormal pupillary response, <br /> ■ changes in speech pattern, <br /> ■ headaches, <br /> 5-3 REV 4/92-EGI <br />
