Well Destruction Groundwater monitoring wells MW-1 through MW-5 were over-drilled with eight <br /> Method: (8) inch diameter hollow stem auger drilling equipment. Over drilling of the <br /> monitoring well was accomplished by centering the drill rig tooling directly over <br /> the center of the monitoring well casing, using a special guide tool that also <br /> grinds up the well casing, maintaining a level and consistent drilling out of the <br /> existing well annular space, and then confirming that the well construction <br /> materials were present in the drill cuttings. The well caps and locks were <br /> removed from each well prior to the over drilling. The borehole was measured <br /> to ensure total depth had been achieved and then neat cement grout was <br /> tremied through the drill augers from the bottom of the borehole to <br /> approximately 3 feet below ground surface. Native material from the site was <br /> used to backfill the remaining void in the borehole to the surface. The bin was locked and secured each night and until delivery to the <br /> landfill for disposal. Representative soil samples were collected for the purpose <br /> to characterize the waste for acceptance with a local landfill. Analytical results <br /> are presented in Appendix C. On November 21, 2013, the soils bin was picked <br /> up by Belshire Environmental Services, Inc. (Belshire)for transportation and <br /> disposal at Potrero Hills Landfill, located in Suisun, California. The soils bin <br /> was transported under an executed Bill of Lading/Manifest(for soils). Disposal <br /> location is documented on the manifest and is included with the analytical <br /> report in Appendix B. No groundwater was generated during these well <br /> destruction activities. <br /> GEOTRACKER DATA ENTRY AND DWR FORMS <br /> Required documents have been uploaded into the GeoTracker database. GHD will also upload a PDF <br /> copy of this report to the GeoTracker database upon completion. A State Department of Water <br /> Resources (DWR) Form 188, Well Completion Reports, have been completed and submitted to the <br /> appropriate agencies for the destroyed wells.
