June 18,2015 <br /> Environmental Health Department, <br /> San Joaquin County, <br /> Stockton,CA 95205. <br /> Dear Sir/Ma'am, <br /> Greetings! <br /> Our facility,ABC Food Mart (CERS ID:10184731) located at 713 N EL Dorado st,Stockton CA 95202 had a <br /> routine inspection for operating permit by inspecting officer Stacy Rivera on May 21, 2015. Please find <br /> below the list of corrective actions taken by us in regards to the violations observed by the officer. <br /> Item: 103:Current Financial responsibility documents not submitted. <br /> Corrective Action: <br /> Required documents are updated and uploaded into the CERS on May 21, 2015.The same document is <br /> also attached here for your reference. <br /> Item: 125: Failed to submit a written response within 30 days of receiving inspection report. <br /> Corrective Action: <br /> Following the inspection on May 2,2014, a written statement was submitted to officer, Ms.Thuy Tran in <br /> person at the county's office, along with pictures for water-tight lid installed on the vent sump. <br /> Following to that,officer requested that the lid used to obtain another lid,as the lid used at the time of <br /> submission of written statement did not fit perfectly. <br /> As a result, a new lid was installed on the vent sump, and the pictures of the same were submitted to <br /> the office of EHD. In addition,the pictures were also emailed to officer, Mr.Jeff Wong <br /> (iwong@sicehd.com). Please find the copy of the email submitted. <br /> That being said, a water tight is installed on the vent sump currently, and attached are the pictures for <br /> the same. <br /> Item 201: Failed to maintain alarm logs and/or records of follow up actions: <br /> Corrective Action: We will maintain alarm history for every occurrence of alarm from now on. <br /> Appropriate actions will be logged and records maintained at the site. <br /> Item 207: Designated operator did not inspect all required items and/or the inspection reports not <br /> completed: <br /> Corrective Action: <br /> Special attention would be ensured to maintain the maintain alarm history reports in the environmental <br /> folder during each monthly inspection.Also,each alarm will be documented appropriately at the facility. <br /> Item 208: Designated operator monthly inspection reports not maintained for last 12 months. <br /> Correction Action: <br /> Copies of twelve months of designated operator monthly inspections will be maintained on site from <br /> now on. <br /> Page 1 of 2 <br />
