Item 209: Designated operator employee training not performed or log not kept. <br /> Correction Action: <br /> An employee training was performed by designated operator on January 15,2015. However,the log was <br /> misplaced. A copy of the log was obtained from the designated officer, and is kept on site for records. <br /> Item 306: Monitoring equipment not installed correctly to detect leak at the earliest opportunity. <br /> Corrective Action: <br /> The leak detection sensor was moved to the lowest point by the service technician at the time of annual <br /> inspection.
