No training record was available on site for the last 12 <br /> months. The training record found on site was dated 2011. The designated operator shall train facility employees for <br /> which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. <br /> The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a repeat violation, Class Il. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 91 fill sump sensor was located several inches above the saddle of the sump, and not located to detect a leak at <br /> the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br /> opportunity. The sensor was relocated to the lowest point of the sump by the service technician. Ensure that all <br /> monitoring equipment is maintained to detect a leak at the earliest opportunity. <br /> This is a Class II violation. <br /> 320 HSC 25290.1(c3), 25290.1(c3) Secondary containment not constructed to prevent water intrusion. <br /> Water was found in the 87 fill sump, the 91 STP sump, the diesel STP sump, and UDC 5/6. According to Mr. Patel, <br /> they did not remove the liquid from the sumps because they do not currently have a lid for their hazardous waste <br /> storage drum. According to maintenance records on site dated 11/3/14 and 2/5/15, dispenser 5/6 was damaged after <br /> being hit by a vehicle and was found to be weeping fuel from a faulty shear valve, which was discovered and corrected <br /> while responding to the subsequent Q3 alarm. Secondary containment shall be constructed to prevent any water <br /> intrusion into the system by precipitation, infiltration, or surface runoff. The service technician removed approximately <br /> one gallon of liquid from the four sumps, and returned the sensors to their proper location. Immediately contact a <br /> properly licensed, trained, and certified contractor to address the water intrusion into the 87 fill sump, 91 STP sump, <br /> and diesel STP sump under permit and inspection of the EHD. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by June 20, 2015. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Received by Inspector: Phone: Date: <br /> (initiaQ: STACY RIVERA, Senior REHS (209) 468-3440 05/21/2015 <br /> Page 7 of 8 <br />
