No alarms were <br /> documented on any of the reports. Alarm histories were not attached to the April 2015, March 2015, December 2014, <br /> November 2014, October 2014, September 2014, August 2014, July 2014, June 2014, or May 2014 monthly inspection <br /> reports. The alarm history reports attached to the February 2015 and January 2015 monthly inspection reports <br /> included only the alarms for Q1 and L1, but not the other 16 sensors. During the monthly inspection, the designated operator shall review the alarm history for the previous <br /> month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm <br /> history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 208 CCR 2715 Designated operator monthly inspection reports not maintained on site for at least 12 months. <br /> The February 2015 designated operator monthly inspection reports was conducted on 2/27/15 and the March 2015 <br /> was conducted on 3/12/15 by an uncertified designated operator. The designated operator's ICC certification expired <br /> on 2/26/15 and was not renewed until 3/26/17, according to records on site. Designated operator monthly inspection <br /> reports for the previous twelve months shall be retained on site. The designated operator must have a current <br /> certification at the time of inspection.
