San Joaquin Street <br /> Stockton, CA., 95201 <br /> Subject: Application for Soil Boring Permit/Well Decommission <br /> Former Lika Corporation Sites, Stockton, California <br /> 2041 Navy Drive Site <br /> 1856 Field Avenue Site <br /> Project No. 90189N <br /> Dear Ms. Hinson: <br /> This letter requests for your approval to decommission 17 monitoring wells at two former <br /> Lika Corporation sites in Stockton, California. Enclosed with this letter are two permit <br /> applications, one for the 12 monitoring wells at the 2041 Navy Drive site, and one for the <br /> 5 monitoring wells at the 1856 Field Avenue site. The decommissioning activities were <br /> described in a letter work plan dated May 24, 1993 addressed to Mr. James L. Tjosvold, <br /> Chief, Sacramento Responsibility Party Unit, California-Environmental Protection Agency, <br /> Department of Toxic Substances Control (DTSC), Site Mitigation Branch, Region 1. A copy <br /> of the work plan is attached. In a letter dated November 22, 1993 from the DTSC to Lika, <br /> approval of the work plan was granted for all of the Field Avenue site monitoring wells and <br /> all of the Navy Drive monitoring wells except for MW-14, MW-14D, MW-16, MW-16D, and <br /> MW-30. These wells were placed under the direction of the Regional Water Quality Control <br /> Board (RWQCB). In a letter dated November 3, 1994 from Ms. Wendy L. Cohen (RWQCB) <br /> to Lika, approval was granted to decommission all monitoring wells at the 2041 Navy Drive <br /> site. Photocopies of referenced documents are attached for your review.
