Hinson, R.E.H.S. <br /> Project No. 90189N <br /> May 16, 1995 <br /> Page 2 <br /> summarizing the specifications of each monitoring well planned for decommisssion is <br /> provided in the work plan. <br /> Also enclosed with the completed "Application for Well/Pump Permit"(s) for the two sites, <br /> are two checks for the application fee of$60 and the SJCEHD oversight fee of$234 for each <br /> site. Maps showing the location of the two sites and their respective monitoring wells is <br /> attached to the permit application. <br /> Woodward-Clyde has contracted Spectrum Exploration of Stockton, California to conduct the <br /> well decommissioning activities. The decommissioning activities will be conducted as <br /> described in the approved work plan, and will involve the evaluation of the condition of the <br /> existing monitoring wells, removal of surface well head enclosures, removal of the well <br /> casing, overdrilling of the completed well seal and gravel pack, and bottom to top grouting <br /> of the overdrilled borehole using tremmie pipe methods. If overdrilling is not feasible, <br /> pressure grouting of the well casing in place will be conducted. All decommissioning <br /> activities will be conducted in accordance with SJCEHD monitoring well decommissioning <br /> standards and the State of California, Department of Water Resources, Water Well Standards <br /> for decommissioning of monitoring wells.
