5-21-1998 2:d5PM FROM P. 2 <br /> 11. UST CORRECTIVE ACTION PROGRAM (CAP) REVIEW PROCESS <br /> A. Work Plan Review Process <br /> Work plans submitted following this Guideline will be evaluated by a Registered <br /> Environmental Health.Specialist assigned to the subject site. The evaluation <br /> will determine whether the necessary permit(s) may be issued for the <br /> commencement of the field activities. <br /> .B_ Sites placed within the CAP should be defcned as follows: <br /> •Contamination is believed to be caused from a leaking underground <br /> storage tank (UST) as defined by Health & Safety Code Section <br /> 25281 <br /> *Soil Contamination Only <br /> *Seasonal high depth to Groundwater is greater than 50 Feet below <br /> grade <br /> C. PHS-EHD will perform the role of lead regulatory agency in this program_ <br /> 111. ENVIRONMENTAL ASSESSMENT REVIEW PROCESS <br /> A. Work Plan ,Review Process <br /> Work plans submitted following this Guideline will be evaluated by a Registered <br /> Environmental Health Specialist assigned to the subject site. In the event of documented contamination at the conclusion of the field <br /> activities, PHS-EHD will make recommendation to the responsible party(ies) <br /> and their cmisultant(s) as to the appropriate agency to oversee the nest phase of <br /> investigation. <br /> Page 4 <br />
