We anticipate conducting this field work in October 1998. <br /> Before field work begins, any buried utilities that are present in the parking lot in the vicinity of <br /> the proposed CPT boring and groundwater monitoring well locations will be located and marked <br /> on the asphalt to prevent any contact with the utilities during drilling. The work space will be <br /> approximately 30 feet by 50 feet, covering an area of approximately 150 square feet. The work <br /> space will be cordoned off during drilling to prohibit access by unauthorized personnel. Field <br /> equipment will consist of a drilling rig and support vehicles. At the end of each work shift, any <br /> waste materials generated during the field activities will be removed and the work area will be <br /> cleaned. <br /> When drilling is completed, the CPT boring will be backfilled with a cement grout. The tops of the <br /> groundwater monitoring wells will be below the existing parking lot surface. Traffic-rated well <br /> boxes, approximately 14 inches in diameter, will be installed to protect the newly installed wells. <br /> The well boxes will be flush with the existing parking lot surface and will not interfere with <br /> parking or other operations on your property. Locks will be installed on the wells to prevent <br /> unauthorized access. Upon completion of this field world, the asphalt surface surrounding the CPT <br /> boring and monitoring wells will be restored to its original condition. <br /> It is unclear at this time how long the groundwater monitoring wells will need to remain on your <br /> property. We anticipate that the time period could be on the order of several years. <br /> Once the CPT boring and groundwater monitoring well installations are complete, the groundwater <br /> monitoring wells will need to be accessed for sampling once each calendar quarter, during the <br /> months of February, May, August, and November. Quarterly groundwater sampling events would <br /> require approximately three hours to complete at your location and would be conducted during the <br /> day. A temporary work area of approximately 20 feet by 20 feet around the well boxes would be <br /> needed to access the groundwater monitoring wells to conduct the groundwater sampling activities. <br /> We will coordinate the schedule for this field activity with you well in advance of each <br /> groundwater sampling event. <br /> Before LFR can perform this work, we must obtain well permits from the SIC PHS/EHD. To issue <br /> the permit, the SJC PHS/EHD must have written permission from you to conduct this work. <br /> Therefore, please sign the acknowledgement in the space indicated, transmit the signed document <br /> to me via facsimile, and return the original to mein the enclosed, self-addressed postage paid <br /> envelope. As mentioned, we currently anticipate conducting this field work in October 1998.
