Selected CPT holes were <br /> re-opened for Mr. Infurna and representatives of Levine-Fricke-Recon by Gregg <br /> Drilling personnel. Asphalt and concrete patches were pre-marked and easily <br /> located and adequately removed. The drop in grout, if any, was measured with a <br /> 13 foot piece of rebar steel in cases where a tape measure would not fit down <br /> the hole. Grout drops greater than three (3) feet were re-grouted with a neat <br /> cement mix and allowed to stay open until Mr. Infurna determined that no further <br /> drop in grout had occurred. The patch was reinstalled to match the surrounding <br /> material. Holes with less than a three foot drop were sanded to about three <br /> inches of the surface and immediately re-patched with a matching material. <br /> Based on the noted drops in several CPT holes, it appears that the original <br /> process of topping off the initial grout at the time of some of the boring <br /> placements was inadequate. Of sixteen (16) CPT holes inspected on the first <br /> night, nine (9) had drops in grout greater than 3 feet and three (3) had a drop as <br /> deep as 13 feet to 18 feet (CPT #s 11 , 14, and 22). <br /> CPT holes inspected on the second evening (#s 115, 101 , 102, and 107) did not <br /> exhibit grout drops greater than three feet (most only had a few inches of drop) <br /> and appeared to have been properly grouted at the time they were placed. <br /> Approximately sixty suspect CPT holes were addressed on January 26 and 27 <br /> 1998 by the Gregg Drilling and Levine-Fricke-Recon crews and based on their <br /> constant contact with Mr. Infuma, no inspection was required for January 28, <br /> 1998. <br /> A Division of San Joaquin County Health Care Services <br />
