San Joaquin County <br /> Public Health Services,Environmental Health Division (PHS/EHD) is the delegated agency to implement the well <br /> standards. <br /> PHS/EHD indicated in correspondence dated May 21, 1997,that due to the time of day that on site work must be <br /> performed,PHS/EHD has elected to perform spot inspections of the vapor sampling points and CPT/PPIP work <br /> that utilizes push point technology. FHS/EHD has made numerous attempts to ensure that all CPT/PIPP and vapor <br /> points were grouted to the surface prior to asphalt patching by the drilling contractor,without PHS/EHD final <br /> inspection of each borehole. <br /> PHS/EHD made verbal agreements with Levine-Fricke-Recon ,Gregg In-situ and TEG field staff regarding the time <br /> to ensure that the grout placed in boreholes does not significantly drop prior to the placement of dry cement in the <br /> boreholes and asphalt patching. Once dry cement is placed within the boreholes and asphalt patched,it is more <br /> difficult to access the boreholes to verify that there are no voids between the patch and the grout. <br /> During the June 5/6, 1997 inspection,PHS/EHD requested that TEG remove some of the vapor borehole asphalt <br /> patches,so that grout depths could be observed. The boreholes grouts had dropped greater than 12 inches <br /> beneath the asphalt patches and FHS/EHD requested that the boreholes be topped off and that the drilling <br /> contractor,TEG,check the prior boreholes to verify that there was grout beneath the asphalt patches. A new <br /> procedure was established whereby the contractor would top off the boreholes several times during the evening <br /> before asphalt patching at the end of the shift. On June 10, 1997,PHS/EHD performed a re-inspection and <br /> verified that those boreholes which had insufficient grout depths had been re-grouted and that the new procedure <br /> was observed by TEG. PHS/£HD also performed a spot check of grouts of previous boreholes and found grout <br /> beneath all the asphalt patches which were removed. <br /> On July 1, 1997,following the resumption of CPT/PIPP drilling,PHS/EHD discussed with the Levine-Fricke-Recon <br /> and Gregg In-situ field staff the new procedure which had been agreed upon for grouting with TEG. On July 11, <br /> 1997,PHS/EHD performed a follow-up CFT inspection and a pilot boring grouting inspection and was informed <br /> by another Levine-Fricke-Recon field staff that the drilling contractor,Gregg In-situ,was waiting 30 minutes to <br /> ninety minutes before asphalt patching each borehole. On July 12, 1997,PHS/EHD performed another follow- <br /> up CPT inspection and a monitoring well grout inspection. PHS/EHD observed that Gregg In-situ field staff, <br /> under the supervision of still another Levine-Fricke-Recon field staff, had finished the grouting of FIFF-25 in less <br /> than 30 minutes. Clearly,the agreed upon methodology was not adhered to by Gregg In-situ field staff. <br /> After further verbal communication with Levine-Fricke-Recon staff,it was agreed that a check would be <br /> performed of the field work which was initiated on June 29, 1997 to determine if similar problems existed with <br /> the CPT/PIPP grouting as was observed with the vapor boreholes grouting. <br /> On July 15, 1997,PHS/£HD staff met with Levine-Fricke-Recon and Gregg In-situ field staff to observe the grout <br /> depths beneath the asphalt patches. Numerous boreholes had dropped greater than 3 feet below ground surface. <br /> Pursuant to San Joaquin County Well Construction and Destruction Standards section 13.I7.7 "Exploratory <br /> borings,installed for sampling purposes,shall be completely grouted with appropriate sealing material.
