This does not infer that a void should exist between the asphalt patch and the grout. <br /> PHS/EHD notified Levine-Fricke-Recon Gregg In-situ staff that from this point forward PHS/EHD would be <br /> contacted prior to asphalt patching to make a final inspection of the grout and would be notified when field staff <br /> changes were made. <br /> PHS/EHD soon began having problems with having to make re-inspections of boreholes which PHS/EHD was <br /> informed were ready for final inspection,when in fact,they were not. Levine-Fricke-Recon developed steel plates <br /> which fit over the boreholes so that traffic cones were no longer necessary to leave over the boreholes awaiting <br /> final inspection. The steel plates reportedly satisfied some concerns of the property owner,regarding traffic safety <br /> and visual distractions. The plates also allowed PHS/EHD to make final inspections prior to asphalt patching on a <br /> less frequent basis. However,it seems that Levine-Fricke-Recon had some concerns regarding off site traffic levels <br /> on San Joaquin County roadways and the dangers which the steel plates may pose. <br /> The latest final inspection of new CPT/PIPP boreholes was scheduled to occur during the daytime hours on <br /> November 26, 1998;however,daytime access to perform this check was denied by the property owner. The final <br /> check was subsequently scheduled for December 2, 1997,during evening hours when a three hour minimum <br /> charge is required for FHS/EHD inspections. To date,a notification and verification methodology has not been <br /> established. Finally,PHS/EHD is unsure if staff access to observe the results of permitted activities is restricted to <br /> evening hours only. <br /> PHS/EHD requested that the drilling contractor,Gregg In-Situ and Levine-Fricke-Recon schedule an opportunity <br /> for PHS/EHD observe the grout depth beneath the asphalt patch of PHS/EHD selected boreholes,as had been done <br /> with TEG and pursuant to the San Joaquin County Well Construction and Destruction Standards section 13.17.11 <br /> which states that"Verification shall be made to determine that an adequate seal has been installed by a EHD <br /> representative." PHS/EHD was subsequently provided with a summary of the boreholes which have been <br /> advanced which included a proposed quality control survey. <br /> The re-inspection activities were finally scheduled for November 24, 1997 and after waiting over one and one- <br /> half hour in the evening,Gregg in-situ drilling staff arrived and informed PHS/EHD and Levine-Fricke-Recon staff <br /> that their management directed them to not remove any asphalt patches so that the grouts could be checked. <br /> PHS/£HD has been informed by Levine-Fricke-Recon staff that the CPT/PIPP grout checking would be performed <br /> during the next phase of investigation when the drilling equipment was again regularly on site. PHS/EHD has <br /> agreed to allow sand to be used to backfill boreholes in which the grout has dropped less than 3 feet below ground <br /> surface,prior to asphalt patching. PHS/EHD would hope that final inspections can be scheduled so as to <br /> minimize the amount of re-inspections during the evening.
