Provisions <br /> 1. The Discharger shall notify Board staff a minimum of two weeks prior to the injection of <br /> GZVI. <br /> 2. The Discharger shall provide an alternate water supply source for any municipal, <br /> domestic or other water use, if affected by the Discharger's wastes. <br /> 3. The following report shall be submitted pursuant to Section 13267 of the California Water <br /> Code. In accordance with California Business and Professions Code sections 6735, 7835 and <br /> 7835.1, engineering and geologic evaluations and judgments shall be performed by or under <br /> the direction of registered professionals competent and proficient in the fields pertinent to the <br /> required activities. The technical report specified herein that contains work plans or, that <br /> describe the conduct of investigations and studies, or that contain technical conclusions and <br /> recommendations concerning engineering and geology shall be prepared by or under the <br /> direction of appropriately qualified professional(s), even if not explicitly stated. The technical <br /> report submitted by the Discharger shall contain a statement of qualifications of the <br /> responsible licensed professional(s) as well as the professional's signature and/or stamp of the <br /> seal. <br /> a. No later than 100 days after injection, the Discharger shall submit a report summarizing <br /> the results of the study, including an evaluation of the effectiveness of using GZVI as the <br /> full-scale remediation of VOC contaminated groundwater in the source area at the Site, <br /> the effects of any byproducts on the receiving groundwater, and any other effects of the <br /> in-situ treatment. The report shall also contain both tabular and graphical summaries of <br /> all the monitoring data obtained during the pilot study. <br /> 1. A narrative description of all preparatory, monitoring, sampling, and analytical <br /> testing activities for the groundwater monitoring. The narrative shall be <br /> sufficiently detailed to verify compliance with the WDR, the attached MRP, and <br /> the Standard Provisions and Reporting Requirements. The narrative shall be <br /> supported by documenting flow rates, and total volume of nitrogen and GZVI <br /> injected, and parameters measured. <br /> 2. An assessment of nitrogen and iron discharge to the aquifer, and results of all <br /> sampling; <br /> 3. Copies of all laboratory analytical report(s); <br /> 4. A calibration log verifying weekly calibration of any field monitoring <br /> instruments (e.g.,pH, dissolved oxygen meter, etc) used to obtain data; <br />
