The <br /> discrepancy between the perforated intervals reported on the video <br /> log and the invoice data has not been resolved. Field inspection <br /> of the PVC liner indicated that it was not secure, which may allow <br /> water to enter this well. This well was sampled in December 1990 <br /> or January 1992 and contained 26 µg/l PCE. <br /> 14-1.2 PERMITS <br /> 14-1. 2a Well destruction permits <br /> Well destruction permits shall be obtained by the contractor <br /> from the San Joaquin County Environmental Health Department prior <br /> to commencing the work. The permits shall be obtained at the <br /> contractor' s expense. Full compensation for all work involved in <br /> filing for and securing well destruction permits shall be <br /> considered as included in the contract prices paid for the various <br /> items of work and no additional payment will be made therefore. <br /> 14-1. 2b Site Safety and Health Plan <br /> The contractor shall prepare and file with San Joaquin County <br /> Environmental Health Department a site safety and health plan. The <br /> site safety and health plan shall contain the following <br /> information: project summary, key personnel, hazard analysis, <br /> training requirements, personal protective equipment, environmental <br /> monitoring plan, site control measures, work practices, <br /> decontamination, emergency procedures (including location and route <br /> to nearest hospital) , and documentation. Compensation for all work <br /> involved in preparing and filing the site safety and health plan <br /> shall be considered as included in the contract prices paid for the <br /> various items of work and no additional payment will be made <br /> therefore. <br /> 14-1. 3 ORDER OF WORK <br /> The wells shall be destroyed in the following order: LVMD4 , <br /> LVMD1, LVMD6, and LVMD2 . <br /> 14-1. 4 DECONTAMINATION, CONTAINMENT, SAMPLING, AND DISPOSAL <br /> All down-hole equipment shall be decontaminated between wells. <br /> Water and sediment produced during the destruction and <br /> decontamination processes shall be temporarily contained onsite, <br /> sampled, analyzed for the presence of Purgable Halocarbons using <br /> EPA Method No. 8010 by a DHS certified laboratory, and <br /> appropriately disposed of based on the results of the laboratory <br /> analysis. <br /> 14-1.4a. Soil: All excess soil material that results from <br /> excavation and demolition shall be removed from the site. <br /> Spoil materials disposal shall be in accordance with <br /> California regulations. All contaminated material may be <br /> 6691\SPECS\SECnON.14 <br />
