Since these containers have no hazardous <br /> waste label or accumulation start date and therefore are considered to be on site <br /> longer than 180 days, immediately contact a licensed hazardous waste hauler to <br /> dispose of this waste under manifest and submit a copy of the manifest to the <br /> EHD by December 19, 2011. <br /> Response: All drums were immediately identified with label (see attached form). <br /> These containers of used oil and were picked up by George M Lowry, Inc., <br /> Environmental Services Division using Consolidated Manifest Invoice No. <br /> 21390 (Attached) <br /> 703. An emergency coordinator and modified contingency plan information is <br /> lacking. There must be at least one emergency coordinator on site or on call to <br /> coordinate emergency response measures, and the following information must <br /> be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, <br /> fire alarm; and the phone number of the fire department, unless the facility has a <br /> direct alarm. <br /> * Immediately appoint an emergency coordinator and post the required <br /> information by a phone. A form is provided that can be used for this purpose. <br /> Submit proof of correction to the EHD by December 19, 2011. <br /> Response: See attached form. <br /> 812. Copies of Universal Waste disposal records were not found on site. A copy <br /> of the disposal receipts of Universal Waste, including but not limited to lamps, <br /> alkaline batteries, mercury containing switches, CRT, electronic devices, and <br /> aerosol can wastes shall be retained on site for at least three years available for <br /> inspection. <br /> * Immediately locate a copy of all Universal Waste disposal records for the last <br /> three years, maintain them on site, and submit copies to the EHD by December <br /> 19, 2011 . <br /> Response: We do not have disposal records for universal waste. We will <br /> designate a container, with proper marking, for future collection and disposal. <br />
