-Ow <br /> Phillips 66 Station No. 5449 Page 2 of 2 <br /> 7647 Pacific Avenue September 5, 2014 <br /> Stockton, California 95207 <br /> • Introducing atmospheric air into the saturated zone (approximately forty-five to fifty feet <br /> bsg) at incremental rates of approximately two, five and ten cubic feet per minute for <br /> approximately two to four hours, or as dictated by field conditions, after SVE has <br /> commenced for approximately two to four hours; <br /> • Collecting depth-to-water and total-depth measurements from every observation and <br /> remediation well utilized in the pilot test prior to conducting the pilot test and collecting <br /> depth-to-water measurements from these same wells following the pilot test; <br /> • Collecting groundwater samples from MW3, MW13, MW14, MW15 and MW22 for <br /> chemical analysis of TPH-g, BTEX, MTBE, ETBE, TAME, DIPE and TBA prior to <br /> conducting the pilot test (unless these wells had previously been sampled within two <br /> weeks of performing the pilot test) and following the pilot test; <br /> • Collecting at least three influent vapor samples (one at the beginning, mid-point, and end <br /> of the pilot test) and collecting at least one effluent vapor sample in Summa canisters or <br /> similar containers, for chemical analysis; although the work plan does not state the <br /> analyses of the influent and effluent air samples, the EHD assumes that the air samples <br /> will be analyzed for TPH-g, BTEX and MTBE; <br /> • Notifying the EHD at least forty-eight hours prior to commencing the fieldwork that <br /> includes installing remediation wells and conducting the pilot study; the EHD prefers at <br /> least a ten-day notification; and <br /> • Submitting a report of findings within four to six weeks of completing the fieldwork that <br /> includes the well installation and pilot study results. <br /> Submit a well installation permit application, Master File Record (MFR) form, Licensed <br /> Contractors' Declaration (LCD) form, and appropriate permit/inspection fees. The permit fee for <br /> installing each remediation well is $130 and the Inspection Activity fee is $390, bringing the total <br /> fee to $910.00. <br /> Please hydrate the bentonite chips for at least thirty minutes when installing the four <br /> remediation wells.
