4.3 SAMPLE TRACKING <br /> Each cooler shall contain at least one Chain-of-Custody (COC) form, <br /> and each sample placed in a given cooler shall be listed on a COC form <br /> placed in that cooler. The individual responsible for collecting the <br /> sample shall sign the COC form(s) and shall be responsible for com- <br /> pletely filling out the form(s) prior to shipment to the laboratory. COC <br /> form completion and maintenance will be performed in accordance <br /> with this SOP. <br /> The completed COC form(s) shall list all the samples contained in the <br /> cooler and shall specify the analytical parameters requested for each <br /> sample. The original COC forms will accompany the samples to the <br /> laboratory. Figure 3 is a typical Chain-of-Custody form. <br /> The laboratory will enter the following information on the COC form: <br /> • Name of person receiving the sample. <br /> • Date of sample receipt. <br /> • Sample condition. <br /> • Temperature of samples. <br /> All corrections to the COC record will be initialed and dated by the per- <br /> son making the corrections. <br /> Each COC form will include signatures of the appropriate individuals <br /> indicated on the form. The originals will accompany the samples to <br /> the laboratory, and copies documenting each custody change will be <br /> received and kept on file. Chains-of Custody will be maintained on file <br /> until final disposition of the samples. <br /> 4.4 QA/QC SAMPLES <br /> To identify potential errors, four types of QC samples will be analyzed <br /> by the analytical laboratory. All QC samples are labeled and sent to the <br /> laboratory along with the actual samples for analysis. The collection <br /> and handling of all QC samples will be documented in the field note- <br /> book. The five types of QC samples are described below. <br /> 4.4.1 Trip BIanks <br /> Trip blanks check for contamination due to handling, transport, contact <br /> with other samples during storage, or lab error. A VOC set filled with <br /> organic-free water is obtained from a lab. This set, along with the em- <br /> pty containers, are taken to the field, labeled with company name, date, <br /> A-% <br />
