Applicant shall obtain written approval from the water <br /> purveyor (well owner) and the Department of Health <br /> Services, Public Water Supply Branch. <br /> 2 . Public Water Supply Well (<200 service connections) --less than <br /> 50 ft. or Private Domestic Well--less than 50 ft. <br /> SPECIAL PERMIT REQUIREMENTS <br /> a.
