2. Distribution <br /> A copy of the Contingency Plan and all revisions to the plan <br /> shall be: [40 CFR 264.531 <br /> a. Maintained by the Permittee at the facility; and <br /> b. Submitted by the Permittee to all local police departments, <br /> fire departments, hospitals and state and local emergency <br /> response teams that may be called up to provide emergency <br /> services. <br /> 3. Amendments to Contingency Plan <br /> The Permittee shall notify the Director of all amendments to the <br /> Contingency Plan. The Contingency Plan shall be reviewed and <br /> immediately amended by the Permittee, if necessary, as required <br /> by 40 CFR 264.54, whenever; [40 CFR 264.54] <br /> a. Applicable regulations are revised; <br /> b. The plan fails in an emergency; <br /> c. The facility permit is revised; <br /> d. The list of emergency coordinators changes; <br /> e. The list of emergency equipment changes; or <br /> f. The facility changes in its design, construction, operation <br /> or maintenance in a way that materially increases the <br /> potential for fire, explosions or releases of hazardous <br /> waste. <br /> 4. Emergency Coordinator <br /> a. A trained emergency coordinator shall be available at all <br /> times in case of an emergency as required by 40 CFR 264.55. <br /> b. The names, addresses and phone numbers of all persons <br /> qualified to act as emergency coordinators shall be supplied <br /> to the Director . This list must be kept up to date . <br /> [40 CFR 264.52(d) 3 <br /> I. RECORD KEEPING AND REPORTING <br /> 1. Operating Record <br /> The Permittee shall maintain a -vritten operating record at the <br /> facility, in accordance with and as required by 40 CFR 264.73 and <br /> 264.74.
