SUBSTANTIVE REQU•MENTS -6- • May 21, 1992 <br /> FOR DDRW, SHARPE <br /> (c) Certifications that the treatment system components are properly installed in <br /> accordance with the design. <br /> (d) Qualifications and hierarchy of personnel operating the system. <br /> 5. The Discharger shall submit to the Board, the Department, and US EPA the TPE <br /> report for approval at least two weeks prior to the proposed commencement of FSO of the <br /> SHARPE's treatment system. This report shall document compliance with Discharge <br /> Specifications C.1(b), C.1(c), C.2, and C.3. In addition, this report shall include a <br /> performance evaluation of the SHARPE treatment system, based on the data collected <br /> during the PWSP and the TPE in accordance with the attached Monitoring and Reporting <br /> Program. <br /> 6. The Discharger shall submit to the Board, the Department, and the US EPA the <br /> POP report for approval at least two weeks prior to the proposed commencement of full <br /> scale operation of the SHARPE ground water treatment system. This report shall <br /> document compliance with Discharge Specifications C.1, C.3, and CA. In addition, this <br /> report shall include a performance evaluation of the complete SHARPE GWTS, based on <br /> the data collected during the PWSP, the TPE, and POP (in accordance with the attached <br /> Monitoring and Reporting Program). <br /> The objective of the performance evaluation is to: 1. determine the ability and reliability of <br /> the SHARPE GWTS in meeting the effluent limitations established in Sections B.1 and <br /> B.2, and to develop the FSO operating parameters necessary to maximize VOC removal <br /> from the plume and the extracted ground water. <br /> 7. The Discharger shall submit a technical report to the Board, the Department, and the <br /> US EPA, prior to the end of the fourth month of FSO of the SHARPE GWTS. The <br /> report shall include all analyses results from the first three months of operation as per the <br /> attached Monitoring and Reporting Program, a summary of the data, and a determination if <br /> the influent to and/or effluent from the air stripper contains previously unidentified <br /> constituents of sufficient magnitude either to pose a threat to water quality and/or to affect <br /> the treatment effectiveness of the air stripper. In the event that significant levels of <br /> previously unidentified pollutants are detected, the report shall show how the Anti- <br /> Degradation Policy and Chapter 15's Article 5 have been complied with, including <br /> recommendations on possible SHARPE GWTS modifications and an assessment of the <br /> impacts to the beneficial uses of the receiving water due to the discharge. <br /> 8. The Discharger shall notify the Board, the Department and the US EPA within 24 <br /> hours of any unscheduled shutdown of the SHARPE GWTS. This notification shall <br /> include the cause of the shutdown and the corrective action taken (or proposed to be taken) <br /> to restart the system. <br /> 9. The Discharger shall notify the Board, the Department and the US EPA <br /> immediately, during normal working hours via telephone, and at least within 24 hours of <br /> any spill of untreated water. This notification shall include the size and cause of the spill, <br />
